Assertive Outreach

Provides holistic and comprehensive services
to adults with severe and prolonged mental illness:

  • Psycho-social education about mental illness;


  • Assistance in areas of daily living (budgeting, transportation, banking);


  • Opportunities for community integration (individual and group activities);


  • Referral for comprehensive medical and dental services;


  • A complete array of psychiatric services (medication monitoring, individual therapy, supportive counseling, crisis intervention, discharge planning, education, and ongoing evaluation).
