NCAP offers support and a variety of services to people living with HIV/AIDS and their loved ones. All of our services are CONFIDENTIAL and FREE OF CHARGE.
CASE MANAGEMENT
The case manager will talk with and listen to each client (i.e. anyone affected by HIV/AIDS) and assess what he/she can do to assist the client in maintaining the highest quality of life possible. Some of the many ways the case manager can help clients include finding housing or food, connecting with a physician, telling their loved ones about their illness, or just listening.
Anyone affected by HIV/AIDS can meet with the case manager to discuss personal questions and concerns about the disease, relationships, and/or resources available.
The case manager assists clients in identifying and prioritizing specific issues and concerns, and together, they develop a plan to address these issues.
The case manager is experienced in connecting people with medical providers, entitlement programs, support groups, and other service programs, including the Connecticut AIDS Drug Assistance Program (CADAP) and the Connecticut AIDS Insurance Program that pays clients' COBRA payments.

ASSISTANCE WITH LEGAL AND FINANCIAL PLANNING
Clients can meet with their case manager to develop a plan to help ensure an income, health insurance and housing.
For clients with legal concerns such as writing wills, creating living wills, planning guardianship of children, delegating power of attorney, or selecting a health proxy, NCAP will facilitate the appropriate referrals to the AIDS Legal Network.

CLIENT DROP-IN CENTER
A drop-in center, located at the NCAP office, is open for use by clients. The drop-in center includes a kitchen, TV, meeting and activity area, a small library of HIV related pubications, and a computer with Internet access.

TRANSPORTATION
NCAP has a van purchased through a Ryan White Title II grant that is used for client transportation to medical appointments and support groups on Tuesdays, Wednesdays and Fridays, 9 a.m. to 5 p.m. The NCAP case manager and volunteer coordinator schedule transportation and also help to arrange for volunteer transportation of clients when the van is not available.

CLIENT EMERGENCY FUNDS
Funds are available, to those who qualify financially, for a variety of medical, pharmaceutical, utility costs and other associated costs, such as childcare and transportation. The case manager can also access a statewide emergency fund to cover housing costs.
